By submitting this application, I certify that I have read and will comply with the CHINO VALLEY FLYERS, INC. constitution, by-laws, club rules, field safety rules, and Academy of Model Aeronautics (AMA) Safety Code. I acknowledge that violation of said rules could result in loss of flying privileges and/or membership in the Chino Valley Flyers, Inc. club. I also acknowledge that I will properly maintain AMA membership, and failure to do so will result in suspension of flying privileges. Upon demand of any club official, I will provide proof of current AMA membership. Military membership applications require simple proof of status such as an identification card. All applications are considered PENDING until appropriate dues are received.
